Complete the Membership Application Form Fill out all required fields, including name, home address, email, phone number, and region selection. Indicate your local chapter and city/state.

Select Membership Type and Region Choose your region (Central, Eastern, Southern, or Western). Indicate if you are a Retiree or part of the New Generation of Leaders.

Submit Annual Dues Payment ($15) Ensure payment is for the current membership year (January - December). Complete the application on the SEIU AFRAM website or download the PDF version. Choose a payment option as outlined on the form.

Submit Application and Payment If completing a paper version of the application, attach one copy of the completed membership form with the payment. Submit an additional copy to your Local Chapter.
